WHAT FORMS OF PAYMENT ARE ACCEPTED ?

Pediatrics 2000 accepts most private insurances, Medicaid and Medicaid Managed Care Plans. Sliding fee scales are offered for patients without insurance. Cash and credit cards are welcome.


IF I HAVE A FORM FOR COMPLETION, CAN YOU FILL IT OUT SAME DAY ?

Yes, as long as the patient has had all tests requested on the form and a physical exam done within a year.


HOW DO I REQUEST COPIES OF MY MEDICAL RECORDS?

You have to fill out a release of medical records form and indicate where the records will be mailed to.

 

I  HAVE BEEN GOING TO ANOTHER DOCTOR, HOW DO I CHANGE TO PEDIATRICS 2000?

First, assign one of ourProviders as your Primary Care Physician (PCP) with the insurance company (we can also do this when you come to the office but we don't guarantee the change will be effective the same day).  You can then bring the child to the clinic and we will request the medical records from the other facility.

 

WHAT DO I NEED TO BRING FOR THE FIRST TIME?

The patient's insurance card, immunization records and medical records if available. We recommend a parent come to the first visit and as many visits as possible, including annual exams.


CAN YOU HELP ME APPLY FOR INSURANCE?

 

Yes, we have insurance representatives onsite from Medicaid Health Plans that can help you apply for the first time or recertify yearly.

 

CAN I STILL BE SEEN IF I DON’T HAVE HEALTH INSURANCE?

Yes, you can request the sliding fee scale form and depending on your annual income and family size you may be eligible for a discount.

 

WHAT INSURANCES DO YOU ACCEPT?

HOW DO I CHANGE THE PCP ASSIGNED BY MY INSURANCE?

Call your insurance company member services department, the number is located in the back of your insurance card and request a change.  Changes are usually done before the 15th of each month.

 

HOW CAN I GET A REFERRAL?

You need to see one of our providers first.  You have to specify to the provider if you have any preferences for the specialist or appointment date/time.  The provider will enter a referral in the system and assign it to the referral helpdesk.  They will schedule the appointment (if allowed) and fax/mail the referral to you.

Keep in mind that we are an open access clinic offering walk-in appointments. Please arrive early preferably by 2:00 PM weekdays, (saturdays before 12:00 PM), to ensure timely access to care.

 

CAN MY CHILD BE SEEN AT THE CLINIC WITHOUT PARENT/CARETAKER PRESENT?

Teenagers 14 and older can be seen unaccompanied as long as they have a signed consent by their parent/guardian in their medical record.  An adult family member may bring a patient in.

 

**According to New York law, a minor (someone under 18 years old) has the right to confidential reproductive healthcare. This means that you can been seen by yourself by a healthcare provider for these kinds of visits and unless you give permission to your healthcare provider he/she cannot release your medical information to anyone, including your parents.**
